YHC offers a compassionate admission process for Navajo Nation members and their families seeking substance use treatment. Our Axiom Care admissions team is here to support you every step of the way, beginning with a comprehensive assessment to understand your needs and connect you with the right level of care.

What to Expect
Taking the first step toward recovery can feel overwhelming, but you are not alone at Yideeską́ądi Hózhǫ́ǫ́jí Center (YHC). Our caring Admissions team is here to guide you every step of the way.
Step 1. Complete a brief pre-screening assessment. One of Axiom Care’s experienced and compassionate Admissions Representatives will guide you every step of the way.
Step 2. Review your options together. Based on your assessment, we’ll talk through the recommended Axiom Care programs affiliated with YHC and answer your questions. Some clients begin their journey at Axiom Care’s Ocotillo Campus Observation or Detox Unit.
Step 3. Get the details. We’ll walk you through what to bring, what to expect, and how to prepare.
Step 4. Schedule your intake appointment at a time that works for you. Need a ride? We can help arrange transportation.

Required Documents
When coming to YHC for admission, it’s important to bring the necessary documents to ensure a smooth and timely intake process. If you’re unsure about what to bring or need help gathering documents, our admissions staff is here to assist and guide you every step of the way. Here is a list of documents that you will need:
1. Proof of Tribal Enrollment
2. Health Insurance (AHCCCS Card, Medicaid)
3. Driver’s License/Photo ID
4. Any referral documents, if applicable (from school, work, social services, etc.)
Insurance Verification
We understand the cost of substance use treatment is an important factor when seeking help.
YHC works with the following AHCCCS providers, but no Navajo Nation member will be turned away due to their insurance status.
